If your insurance includes out-of-network benefits, I can provide you with documentation (a “superbill”) to submit to your insurance for reimbursement. It may be useful also to know your out-of-network deductible and co-payment responsibility, and in particular what your insurance reimburses for CPT codes 90792 (intake), and 99214 (follow up) with or without psychotherapy (90836).
